The annual renewal period for 2026-2027 is now open! All registrants must complete the Practice Update Form and submit their renewal fee by June 1* through the Member Portal.
*The membership fee for Supervised Practice is paid in two installments: June 1 and December 1 of each year.

Fees are due on or before June 1. A 20% late fee is charged for renewals received after June 1. Your invoice is available in the Membership Services area.
How to complete the Practice Update Form
- Make sure all information is accurate in the Complete sections.
- Complete any section marked as Review.
- Submit the Form — you’ll be directed to a secure payment page.
Changing Your Certificate?
If you plan to switch your status, please submit your Certificate Change Request Form before renewing. The form can be found in the Member Portal, under Certificate Change/Resignation in the left menu.

- Can you request a change now to take effect later? Yes. For example, if you plan to go “Inactive” in September, you can submit your request now.

New this year: voluntary demographic questions
CPBAO collects practice, education, and some demographic information on the Practice Update Form. Much of this data is requested by the Ontario Ministry of Health and HealthForceOntario to support health human resource planning and is collected across all regulated health professions.
Starting this year, CPBAO is also including a small set of voluntary demographic questions as part of the renewal process. This is part of CPBAO’s Ethics and Human Rights Plan and is intended to strengthen our ability to serve the full diversity of the Ontario public.
An essential component of this work is improving the quality of information we have about the professions. Better data helps identify potential barriers and strengthen fairness in our processes.
Responses are anonymous and will not be linked to your registrant profile. All responses are de-identified when stored and will be used only in aggregate.
